To get the phone off the black list you’ll need to payoff the carrier for the rest owed on the contract as the carrier didn’t get paid for the phone. Once paid you should be able to get them to also unlock the phone. In either case you’ll need to visit the carriers brick & mortar storefront for them to help you.
